What is Retinopathy Prematurity ?

A condition known as retinopathy prematurity (ROP) causes abnormal blood vessels to

form in the developing retina of premature babies, resulting in blindness. (the cornea’s

deepest, light-sensitive layer). In a full-term infant, the blood vessels come to an end along the

surface of the growing retina. Premature newborns have inadequate growth and maybe aberrant

vascular growth. These atypical blood arteries are brittle and prone to bleeding. Bleeding repeatedly

can leave scars. Retinal detachment results from the scar tissue’s pulling on the developing retina as it contracts.

What is Retinopathy of Prematurity Screening ?

Within the first 28 days of life, babies weighing less than 2 kg and born before 34 weeks should be checked for ROP. Usually, an ophthalmologist with similar training performs this. Serial exams are performed every week or every two weeks until the growth is finished or to see whether sight-threatening ROP is occurring. 

What are the symptoms of Retinopathy Prematurity?

ROP has no symptoms. Parents frequently don’t realize their baby is blind until the child is between 6 and 8 months old, or sometimes even older. Thus, screening is crucial for premature infants. Less severe ROP variations could be linked to: 

  • Abnormal vision 
  • Eyes crossed and squinted 
  • Extreme myopia 
  • The pupil’s white reflex 

What are the Risk elements related to ROP-Prematurity?

  •  Extended oxygen requirement

What are the ROP Stages ?

There are five stages to ROP : 

  • There are five phases of ROP, each getting more severe. Regression can occur in stages 1 and 2. 
  • Treatment is typically required for stage 3, or sight-threatening ROP. 
  • The most severe stages are 4 and 5, and even with treatment, they frequently have poor visual outcomes. 
  • The word “plus disease” denotes a more serious ROP. 

What are the Zones of Retinopathy of Prematurity?

The retina of an infant is categorized into three zones, namely:

Zone 1, which is the most crucial for vision, Zone 2, which requires treatment in stages 3 and higher, and Zone 3, which typically does not require treatment.

What is the Treatment for Retinopathy of Prematurity?

The cornerstone for managing sight-threatening ROP is laser photocoagulation. ROP disease in stages three plus needs to be treated. Surgery, either a vitrectomy or scleral buckling, is required for stages 4 and 5. In certain instances with Zone 1 disease, particularly in very ill infants who are not able to undergo laser photocoagulation, anti-VEGF injections may be administered.
